About This Service
استخراج شهادة الميلاد المميكنة من السجل المدني، سواء كانت لك أو لأحد أفراد أسرتك، وسواء كانت أول استخراج أو صورة إضافية طبق الأصل.
تحتاج إلى بيانات صاحب الشهادة: الاسم والرقم القومي وتاريخ الميلاد ومحل الميلاد واسم الأب واسم الأم، مع تحديد صلتك بصاحب الشهادة. المستندات المطلوبة هي بطاقة مقدم الطلب، وصورة من شهادة الميلاد المطلوبة إن وُجدت، وصورة القيد العائلي.
نقدّم الطلب ونتابعه لدى السجل المدني حتى استلام الشهادة، ثم نسلّمها بالطريقة التي تختارها. المدة التقديرية ٣ أيام عمل.

Check your details and documents, then send the request to the office.
4 Declaration Form
Your request has been sent. All that remains is the declaration of accuracy that completes the authorisation.
